Please bring your own mask. We cannot provide masks for you.
When you arrive, please remain in your vehicle. We will call your phone to let you know when we are ready for you to come up to the lobby.
No guests of the patient (spouse, family, friends, etc.) are allowed to wait in the lobby while the patient receives treatment.
We will ask you to wash your hands when you arrive at the operatory.
We will ask you to do a pre-procedural rinse and will also take your temperature.
Have you or a member of your household traveled outside of Alaska in the last 14 days?
Have you or a member of your household been ill in the last 14 days (fever, cough, shortness of breath, or other respiratory problems, loss of sense of smell or taste)?
Have you recently been in close contact with someone who has tested positive for COVID or someone who has had any of the previously stated symptoms?
If a patient answers “yes” to any of these questions, they should not be seen at least 14 days from last symptom or contact.
